Case Study Report Problems - 2000

10. The patient is a 40 year old construction worker who is referred for physical therapy with a diagnosis of "rule out discogenic pain - evaluate and treat".  He gives a history of gradual onset of pain at the buttock and lateral aspect of the hip without trauma.  He denies low back pain.

12. The patient is a 35 year old secretary who complains of back pain with increased activity (she first noticed the pain while jumping up and down in aerobics classes which she started a few weeks ago).  Her prescription reads "low back strain - evaluate and treat".  Your examination reveals exaggerated lordotic posture (standing and sitting) with limited active and passive forward bending mobility.

16. The patient is a 25 year old woman who was involved in an automobile accident two days ago, and is referred with a referral that reads, "Whiplash injury with cervical strain - evaluate and treat."

20. The patient is a 40 year old man who is complaining of shoulder pain.  This developed after playing the opening game of the season for the company softball team two days ago.  The referral reads, "Shoulder tendinitis - evaluate and treat."

21. The patient is a 30 year old woman who complains of lateral arm pain near the elbow.  She reports that this developed after helping her husband build a deck on the back of the house during the past weekend.  The referral states, "Elbow pain of musculoskeletal origin - evaluate and treat."

24. The patient is a 40 year old male who complains of pain in the hand.  He does not recall any trauma.  His physician feels the problem is musculoskeletal in origin.  The referral reads, "Hand pain - evaluate and treat."
