INDUSTRIAL PT
(aka Occupational Health PT)
CASE REPORT GUIDELINES

The purpose of the case report is to allow the student to show an understanding of the Industrial PT process. Case reports should include the following:

  1. Case Introduction -- State the relevant details of the client's work, medical and rehabilitation history up to the point when the client was referred to you. Include the referral source.
  2. Job Description -- Present the job description for the job the client held when s/he was injured. Use the guidelines for the Job Description assignment to develop the job description.
  3. Functional Capacity Evaluation (FCE) -- Give a detailed report of what was done and what were the results (you create the FCE results based on a reasonable projection from the facts stated in the introduction).
  4. Work Conditioning / Work Hardening Goals -- List the goals you wish to achieve based on the data presented in items 1 - 3.
  5. Work Conditioning / Work Hardening Plan -- Produce a detailed plan of action to reach the goal(s) you establish for the client.
  6. Report Letter to Referral Source -- Write a report letter using acceptable documentation formats. Include a brief review of client history, results of FCE, goals and plan.

Format:

As with all written work for PT Department assignments, use AMA format and style (see author instructions posted at the web sites for Journal of the American Medical Association and Physical Therapy). Type and double-space the report. Number each page. Start with a title page that contains the words "Industrial PT Case Report," the case number and title, the date, and your name. Start each of the sections listed above on a separate page. The format for the report letter should be appropriate for professional correspondence. Single-space the report letter. The finished report should be of the same quality as a manuscript submitted for publication in a professional journal. Spelling, grammar, style, etc. will be considered when grading your report.

Demonstration: Be prepared to give an oral report about your case and demonstrate some activity from the FCE or work conditioning/work hardening sections of your case during the last laboratory period.
