Case Management/Critical Pathway Assignment

        The purpose of this assignment is the develop a case management plan and critical path Case management is a system of client care delivery that focuses on the achievement of outcomes within effective time frames and with appropriate use of resources. It encompasses an entire episode of illness, crossing all settings in which the client received care (Zander, 1989).

        Case management plans are time-lined, standardized plans of care for clients in a specific diagnostic case type. A critical pathway is developed for each major case type and serves as an abbreviated version of the case management plan. Critical paths demonstrate critical or key incidents that must occur in a predictable and timely fashion to achieve an appropriate length of stay and provide cost effective care.

Directions: This assignment includes a short formal paper (5-7 pages) that provides additional background for the case management plan and the critical pathway. This assignment is due to your clinical instructor on May 7, 1999.

All components of the assignment must be completed in order to receive credit for the entire assignment. 
 
 
The case management plan is developed as follows: 
Select a target case type (diagnostic population). Target case types may be selected by parameters such as the largest case types by volume and/or cost, or case types that are problematic. Target case types require similar care and resources.
Identify providers of care who are expert in caring for clients with selected target case types.
Complete an assessment base for the case type utilizing the audit and the literature review.
Identify patient-centered goals for each problem.
Identify measurable outcome criteria for each goal
Identify time frames within which care will proceed. The time frames include the expected length of stay within all areas in chi the patient will receive care (e.g. in-patient, or out-patient) as well as the processes and intermediate goals for each specific time period involved (hour, shift). The time frames established in the phase allow the caregivers to focus on the parameters for each geographic area since case managed care is unit-based.
Identify specific nursing and other health care professional interventions that are necessary to achieve the client centered goals. Be sure to state times for interventions in order to facilitate the sequence of critical events to ensure are is effective and cost effective.
Identify variances (difference between expected care and actual care) that become evident. Sources of variances include the system, clinician or the client.

 
The critical path is an abbreviated case management pan that is developed from the information gathered for the case management plan.
